Assisted Living Apartments

What We Offer

Our program offers long-term housing with supportive services to adult men and women living with serious mental illness. This program enables tenants to maintain stability, thereby enhancing the quality of their lives and furthering their capacity for recovery.

In each of our three- or four-bedroom apartments, tenants share a living room, bathroom, and small kitchen and sleep in their own bedrooms. They help each other by sharing their problems and living cooperatively.

Our approach provides an individualized schedule of activities for each tenant that includes opportunities for work, volunteering, and other involvement in the surrounding community.

Daily, our staff prepares and serves tenants a nutritious lunch and dinner in a congregate dining area.

We support achievement of an individual’s goals, including

  • Helping the tenant to build his or her skills in illness and
    symptom management
  • Supervising the individual's medication and health care
  • Helping the consumer with personal care
  • Educating the individual about health and nutrition
  • Providing the tenant with the opportunity to participate in regular meetings and groups
  • Helping the tenant to develop housekeeping skills, including budgeting, cooking, apartment cleaning, laundry, and grocery shopping, with assistance to persons who are unable to complete these tasks on their own
  • Offering the consumer the opportunity to participate in recreational activities and field trips
  • Providing the tenant with an integrated approach to
    co-occurring mental illness and substance abuse

We provide these supportive services 24 hours per day, seven days per week.

About Our Staff

Respectful and caring, our staff takes the time to understand what is happening in tenants' lives. Through our support, individuals develop the skills they need to live fulfilling lives and the resources they need to support their goals.

Our staff possesses diverse skills, with education and experience in a variety of fields, including social work, psychology, chemical health, therapeutic recreation, and nursing.

Our staff includes:

  • Mental Health Coordinators
  • Mental Health Assistants
  • Registered Nurses
  • A Food Manager
  • Dietary Assistants
  • An Activity Coordinator
  • An Office Manager
  • The Director of Supportive Housing

Admissions

To qualify, a potential tenant must have a mental illness diagnosis, have an assigned case manager, and meet the criteria for CADI services (Community Alternatives for Disabled Individuals).

We are registered with the Minnesota Department of Human Services as a Housing with Services Establishment and are licensed by the Minnesota Department of Health as an Assisted Living Home Care Provider. The program operates under a contract with Hennepin County.

The following resources cover the cost of our services:

  • Medical Assistance or an individual’s other resources cover CADI services, TBI (Traumatic Brain Injury) services, and ARMHS (Adult Rehabilitative Mental Health Services).
  • Each tenant pays his or her share of the rent, congregate meals, and utilities.
