Intentional Communities Supervisor
We are seeking a full time Supervisor to provide leadership and management for the intentional communities. Supervisor will coordinate admission and discharge process and conduct initial comprehensive assessment of clients to determine needs. They also complete individual plans with clients and IC team members and make recommendations to team. Must also be able to communicate instructions and methodologies as appropriate to ensure that plan is implemented correctly. This position functions as a part of the leadership team across the agency. Ability to meet the Minnesota Department of Human Service Rules for qualifications of a mental health professional required along with experience working with people who have a diagnosis of SPMI, knowledge of evidence based treatment practices, and supervisory experience.

Care Guide
The Care Guide is responsible for working with clients enrolled in Cornerstone Solutions to help them access, navigate and coordinate medical, mental health, oral health, chemical health and social service related systems and services. This position will work as an advocate for the consumer across a variety of county agencies and will be responsible for developing and maintaining relationships with providers and community partners. Must be able to meet the MN DHS qualifications for a mental health practitioner and have experience working with adults with a certified disability. A master’s degree in the social sciences is preferred.

Mental Health Assistant
This part time position works at our Assisted Living Facility for adults with mental illness. They work with residents on ILS, provide crisis management, document observations and supervise medications. Must be flexible with schedule and would primarily work evenings, weekend and some overnights if needed. Generally 4-14 hours a week. Must be at least 18 years of age, and possess a high school diploma or equivalent. Experience working with people who have severe and persistent mental illness preferred.

Office Manager
This position is primarily responsible for managing the accuracy of electronic client records, insurance, and authorization information. Also provides administrative support services to staff to ensure multiple program effectiveness and efficiency. Responsible for establishing a welcoming, professional environment for clients and others visiting or calling the office. Position requires a high level of accuracy, attention to detail, and effective communication skills.

Assisted Living Director
We are seeking a full time Director to work in our residential program serving 19 adults with mental illness. This position provides an array of services that support client recovery and facilitate safe, secure, supportive intervention and crisis prevention. This position also provides supervision and leadership for mental health services. The Director will have responsibility for the Mental Health Coordinators, the Food Service Manager, and the Registered Nurse. This position functions as a part of the leadership team at the Assisted Living and across the agency. Applicant must meet the Minnesota qualifications of a mental health professional in addition to having experience with adults with SPMI and organizational leadership.

Certified Peer Specialist
We are seeking a part time Certified Peer Specialist (CPS) to work at our residential treatment facility. The CPS will work in the community with recipients of the program to assist them in setting and achieving rehabilitation goals, securing employment, completing paperwork, providing mentorship, and other duties as assigned. Applicant must be able to pass a DHS background study and provide proof of certification. Applicant must also be self-identified as a current or past recipient of mental health services. Position requires a varied schedule including some day, evening and weekend hours.

Food Service Assistant
We are seeking a part time (16 hours per 2 week pay period) Food Service Assistant to work in our residential program serving 19 adults with mental illness. Mainly weekend and holiday hours with some flexibility to work days or evenings if needed. Position is responsible for client meal preparation and clean up. Must have high school diploma or equivalent. Current ServSafe certification or equivalent is preferred.
