Recovery from mental illness and whole person wellness starts at Touchstone Mental Health where our mission is to “inspire hope, healing and well-being.”
We consider each person as a whole. We integrate treatment, care and resources which support a healthy mind, body, and spirit. We are a point of reference guiding clients, community members and residents toward recovery and well-being. We encourage each person to define their life goals— physical, mental, emotional, spiritual, social, vocational and interpersonal—and we provide the tools so people achieve them.
Respectful and caring, our staff takes the time to understand what is happening in each person’s life. They possess diverse skills, with education and experience in a variety of fields including social work, nursing, psychiatry, psychology, counseling, chemical health, therapeutic recreation, nutrition, healing arts and marriage and family therapy.
Across our programs, we serve 450 adult women and men annually, supporting their lives in our communities.

We use the latest in evidence-based practices as well as innovative approaches to help create success for those we serve.
Complementary Service Offerings
Across our programs, Touchstone offers community members, residents and clients the opportunity to participate in complementary service offerings. Touchstone currently offers Acupuncture, Healing Touch, stress reduction, guided imagery, mind body skills training, as well as Mindful Movement classes.
Complementary services provide positive and sustaining effects which assist with sleep deprivation, depression, agitation and anxiety, as well as help to manage addiction, stress and chronic pain. Over the past four years, participants have expressed feelings of increased relaxation, positive shifts in mood and an increased sense of physical well-being.

Summary of Programs
Touchstone Assisted Living Apartments
Minnesota’s first long-term assisted living program for adults 18 and older with serious mental illness provides residents with 24-hour supportive services so they can enjoy life’s opportunities.
Our capacity is 19 residents.
We offer:
- 3- or 4-bedroom apartments with shared living rooms, bathrooms, and kitchens.
- Individualized illness and symptom management skills assistance as well as medication and health care coordination management assistance as needed
- Individualized schedule of activities, including opportunities for work, volunteering, and other involvement in the surrounding community.
- Nutritious lunch and dinner served in a congregate dining area.
- Meditative and mindful movement classes, step-up exercise program and photography expressions are also available to all residents.
Admission qualification: Diagnosis of a serious mental illness; service need is 24-hour care Medical Assistance and CADI or TBI waiver allocation; or other funding to pay for the needed services.

Touchstone Intensive Community Rehabilitative Services & Targeted Case Management
Touchstone provides individualized services for adults age 18 and older with a diagnosis of serious mental illness. Case management services are geared to help with gaining access to needed medical, social, educational, vocational, financial and other necessary services, while intensive rehabilitative services help with developing and enhancing strengths, psychiatric stability, social competencies, personal and emotional adjustment, independent living and community skills by working with you on a weekly basis, if necessary. As a result, adults are able to remain living in the community, minimize the risk of hospitalization and improve their overall functioning and health. Our capacity is 120 adults.
We offer:
- Meeting with you one or more times per month and offering help with all aspects of independent living, connecting to services and resources and/or symptom management.
- Team of mental health professionals, including a psychiatrist, who support you.
- Personalized community support plans.
- Holistic or complementary health practices, including healing touch and acupuncture.

Touchstone Intentional Communities
Minnesota’s first housing support and gathering structure of its kind for adults living with mental illness throughout Hennepin County. Community members come together as a supportive group, sharing a common purpose and working cooperatively to create a lifestyle that reflects shared values. Through Intentional Communities individuals are able to maintain housing stability, reduce the risk of hospitalization, gain independent living skills and integrate their lifestyle choices within the greater community. Our capacity is 40 community members.
We offer community members:
- Centrally located apartment for each community that provides space for members to meet, plan recreational activities, and prepare and share communal meals.
- Touchstone staff serve as organizers and advocates for the two communities to assist with coordinating members’ activities, and providing support.
- Rehabilitative services to help with developing and enhancing psychiatric stability, social competencies, personal and emotional adjustment, independent living and community skills.
- The self-governing communities may choose to participate in acupuncture, yoga, meditation or other holistic health practices.

Touchstone Residential Treatment
Residential treatment offers short-term, intensive residential treatment services for up to 90 days. The residence is staffed 24 hours a day, seven days per week, to assist individuals with continued symptom stabilization as well as provide an opportunity to increase independent living skills necessary to assist with successful reintegration into the community. We serve 14 men and women over the age of 18 that carry a serious and persistent mental illness diagnosis and could also have substance use disorders.
We offer:
- A supportive, structured, and comfortable environment that focuses on the integrative services that include the mind, body, and spirit.
- Individualized treatment plans, individual counseling, psycho-educational groups, and the opportunity to practice and improve independent living skills.
- Supportive staff assistance with financial, recreational and other needed services within the community.
- Holistic or complementary health practices, including acupuncture, mindful movement groups, meditation, and guided imagery available to all residents.
Funding Sources Include: Medical Assistance, Intensive Residential Services referral from Hennepin County, or private medical insurance.

Touchstone Care Coordination
Touchstone Care Coordination provides integrated care for people with disabilities by serving as a central contact point to help you access social services and health care services. Touchstone’s Care Guides assist in navigating the health care system and setting up services, including coordinating transportation to medical appointments for clients of Metropolitan Health Plan’s Cornerstone Solutions. We currently support more than 200 Cornerstone Solutions members.
We offer:
- Comprehensive health and risk assessments and individualized care plans which enable clients to receive the appropriate medical, mental and social services.
- Coordinating with social and human service, medical and mental health professionals to meet each client’s needs and assist with maintaining appointments.
Admission qualification: Must be a plan member of Metropolitan Health Plan’s Cornerstone Solutions.
