Program Description
Touchstone Care Coordination operates with mindful intention to honor the mission of Touchstone Mental Health. We inspire hope, healing and well-being through our integrative services linked directly to therapeutic and skill building intervention promoting specific and broad outcomes for our clients. We work with people to meet their bio-psychosocial needs which include physical and mental wellness, vocational and educational development, as well as leisure and recreational fulfillment.
What We Offer
Touchstone Care Coordination is contracted to provide Care Guide services for Metropolitan Health Plan’s Cornerstone Solutions members. We serve as advocates for our clients across the spectrum of physical, mental and social service providers. Our integrated approach includes an initial holistic health consultation which assesses each person’s preventative health, mental health, oral health and social service needs. Based on this assessment, we work with each person to develop a care plan and assemble a care plan team of professionals which helps ensure all appropriate medical, mental and social service needs are met. Care plan progress is reviewed periodically throughout the year.
Program Team members
Our team is comprised of mental health professionals and practitioners who work in collaboration with Hennepin County and Metropolitan Health Plan Cornerstone Solutions providers. Team members possess diverse skills, with education and experience in a variety of fields, including social work, psychology, chemical health, nursing, and health & wellness.
Admissions
Individuals interested in receiving Care Coordination Services must be enrolled in the Metropolitan Health Plan’s Cornerstone Solutions insurance plan. To inquire about eligibility in Cornerstone Solutions call MHP at 1-866-601-8962.
