Program Description
Touchstone’s Intensive Community Rehabilitation and Targeted Case Management Programs’ multi-disciplinary team approach offers clients a support system dedicated to their individual physical, mental, social, and vocational needs. The rehabilitative model guides team members as they help clients cultivate skills to maintain psychiatric stability while residing in the community.
What We Offer
Team members offer a wide range of services such as medication management, monitoring of chronic medical conditions, and skill building to promote self-management of mental illness symptoms as well as independent living in a community context. Also available are complementary health practices including Acupuncture, Healing Touch, stress reduction and mind body skills training.
Clients meet with various members of their team on a weekly basis to address goals that have been previously determined through a collaborative process of assessing client needs and aspirations. Clients may meet with team members to learn such skills as managing their illness, finding housing, creating budgets and enhancing vocational skills.
The overall experience of the client is one that emphasizes personal choice and resilience in one’s life.
Program Success is measured on:
- Decreased use of hospitals for crisis or symptom management
- Stable housing and independent living tenure
- Avoiding civil commitment
- Fulfillment of personal goals
Program Team members
Each client works with a multi-disciplinary team comprised of team members that possess diverse skills, with education and experience in a variety of fields, including psychiatry, social work, psychology, chemical health, nursing, complementary health practices, and health & wellness.

If you have answered yes, our ICRS/TCM Team might be a good fit for you! Admissions are based on referrals from Hennepin County.
