Touchstone Programs
Touchstone programs help people recover from mental illness and develop the skills they need to live well in their communities. At Touchstone, we consider each person as a whole; integrating treatment, care and resources which support a healthy mind, body, and spirit.

Behavioral Health Home
Behavioral Health Home (BHH) is a community program that can help you find services and coordinate your care for physical and mental health conditions. Our staff will meet you in your home or in the community to create a plan that meets your needs and preferences.

Residential Treatment Services
Residential Treatment offers 24-hour mental health services in a structured, supportive environment designed to improve mental health stability, increase personal and emotional skills, self-sufficiency and independence.

Fitness Center
The Fitness Center is available to anyone 18 or older who is interested in improving their physical fitness. The Fitness Center offers a variety of weights, machines, and personalized fitness in a small community gym. Individual or group memberships are available at an affordable rate. It is located in the Seward neighborhood in Minneapolis.

Housing Innovations
Housing Innovations helps people find and move into community housing and offers ongoing support that help people maintain their housing. Independent living support, wellness education and social activities are also available for people who are approved for waiver services.

Intentional Communities
The Intentional Communities program provides in-home services to build social, community and housing management skills. Program members meet weekly as a “community” of peers who support each other in their mental health recovery.

Case Management
Our Case Managers help people with disabilities and mental illness form short and long term goals, find resources and referrals, monitor their health and safety, and provide ongoing support for clients to improve their independence and remain in their homes.

Care Coordination
Touchstone Care Coordination helps people navigate our complex health and social service systems. Care Coordinators develop a care plan and work closely with the person’s primary care, mental health care and social service providers in order to achieve improved health.

Rising Cedar and New Hope Apartments
Permanent housing with 24-hour onsite services offer people the stability and care they need to focus on recovery and building independence. The program offers nursing services, mental health support and activities.

Minnehaha Commons Apartments
Minnehaha Commons Apartments provides permanent supportive housing with 24-hour onsite services for people ages 55 and older who have experienced homelessness and may have a mental or physical health condition.
