Assisted Living Apartments

Who We Serve

Touchstone’s Assisted Living Apartments in Minneapolis (Rising Cedar) and New Hope serve adults with a serious mental illness who need 24-hour supportive housing and care.

Please note:  The Rising Cedar waiting list is closed and not accepting new applicants.

Assisted Living Admissions

Assisted Living referrals typically come from a Case Manager through Hennepin County. Please see the referral pages for more information about the referral process.

Touchstone Mental Health’s Assisted Living programs have embraced the American Health Care Association and National Center for Assisted Living (AHCA/NCAL) National Quality Award Program. The program is based on the criteria and core values of the nationally recognized Baldrige Performance Excellence Program and includes three progressive levels (Bronze, Silver, Gold). Member organizations can apply for recognition.

We celebrate that both our Touchstone Mental Health communities have been recognized with the Bronze – Commitment to Quality Award!
