Care Coordination

Joanne came to Care Coordination having schizophrenia, multiple health conditions, including diabetes, and a history of substance abuse. Touchstone’s Care Coordinator worked with her to get a release of information to help in making appointments, arranging transportation, and helping her follow up with her doctor, psychiatrist, and diabetes nurse. Now Joanne is managing her diabetes through frequent contact with her medical providers.

Once she started managing her diabetes, her Care Coordinator was able to help arrange for additional supports like in-home nursing services, independent living skills training and homemaker services. Joanne also visited a community support program with her Care Coordinator and now attends on a regular basis.

Joanne is proud to say she feeling better, has been sober for over a year and doing well in maintaining her apartment.

This is what Touchstone Care Coordination looks like. Our clients receive monthly face-to-face and phone support from someone who can help them problem-solve, ensuring access to health care and insurance, social services, and basic needs like food shelf and work programs.

Who We Serve

Touchstone’s Care Coordination program is open to adults 18 and over who are enrolled in a health care plan contracting with Touchstone Mental Health. Learn more about referrals to Touchstone’s Care Coordination program.
