Rising Cedar and New Hope Apartments Referrals
Program Details
Rising Cedar and New Hope Apartments serve adults with a serious mental illness with 24-hour onsite services that are flexible to meet individual needs.
Services include nursing, mental health and crisis support, housekeeping and meals. The program also offers activities, a community garden, animal assisted therapy, and fitness center opportunities.
Eligibility
To be eligible for Rising Cedar and New Hope Apartments, individuals must be:
- Single adult, age 18 or older
- Diagnosed with a serious mental illness and/or co-occurring substance use with a length of sobriety, with no threat to themselves or others
- In need of 24-hour monitoring and support; must be able to complete activities of daily living (ADLs) and if needed, administer insulin independently
- Eligible for a CADI or BI waiver

Referrals usually come from a Case Manager through Hennepin County. If you need a Case Manager, contact Hennepin County.
