Case Management Referrals

This section is intended for medical and mental health providers. If you are looking for information for you or a family member, please review our Case Management information.

The Case Management program provides resources for adults in Hennepin County to maintain and increase independence, stability, functioning, and well-being. Touchstone provides both Intensive and Targeted Case Management services for clients depending on the client’s level of need.

Provider Referral Information

Providers must fill out and send us the following documents, required by Hennepin County, for all Case Management referrals:

If you are not a service provider and you need help in finding mental health services please contact Hennepin County.

Program Details

Intensive and Targeted Case Management Services

Touchstone’s Case Management services include:

  • Clinical assessment of functioning
  • Treatment plans including rehabilitative services to increase independence, psychiatric stability, independent living skills, and social competencies
  • Dedicated case managers to help clients in connecting with service providers, community resources, housing, and family
  • Access to integrated health and wellness services

Targeted Case Management Eligibility

To be eligible for Targeted Case Management (TCM) Services at Touchstone individuals must:

  • Have a serious mental illness diagnosis
  • Be an adult 18 years of age or older
  • Have a provider or Hennepin county referral

Intensive Case Management (ICM) Eligibility

To be eligible for Intensive Case Management Services at Touchstone individuals must:

  • Have a serious and persistent mental illness diagnosis
  • Be an adult 18 years of age or older
  • Have been hospitalized at least once in the past year; or homeless; or have a history of requiring mental health crisis, criminal justice, civil court in past year
  • Have a provider or Hennepin County referral
