Housing Innovations Referrals

Contact Hennepin County Front Door at 612-348-4111 if you need a waiver assessment or case manager.

Eligibility for Touchstone Housing Stabilization Services

To be eligible for services, individuals must be:

  • Adults with a mental health condition living in Hennepin County
  • Receive Medical Assistance
  • Experiencing Housing Instability
    • Currently experiencing homelessness
    • At risk of becoming homeless
    • Transitioning from a licensed setting
    • Eligible for MA waiver services
  • Needs assistance due to their mental health condition to:
    • Communicate
    • Manage challenging behaviors
    • Make decisions

Provider Referral Information

Referrals for Housing Stabilization Services can be made by a Targeted Case Manager, Waiver Case Manager or Housing Services Consultant.

If you would like to refer an individual to our program, please complete the Referral Form.

Eligibility for Touchstone Home and Community Services

To be eligible for services, individuals must:

  • Adults with a mental health condition living in Hennepin County
  • Need Home and Community Services
  • Be approved for CADI or BI waiver services

Provider Referral Information

Referrals for Home and Community Services can be made by a Waiver Case Manager. If you would like to refer an individual to our program, please complete the Referral Form.
